Complete the attached Transcript Request Form (below) and take it to Mrs. Schellinger, the Registrar, to obtain a copy of your transcript.
Note: the Transcript Request Form must have your parent's/guardian's signature even if you are 18 years old. Make sure your transcript includes your Senior Fall Semester grades, a list of your Senior Spring Semester classes, class rank, and cumulative weighted GPA.
